1. Original Application Form;
2. Original Documentary Requirements prescribed per Board; and
3. Valid Philippine Passport.
Failure to present and/or submit the same may cause the disapproval of application to take the Licensure Examination and non-issuance of Notice of Admission.

- OWWA membership is valid for 2 years and may be renewed on or before the expiration date.
- An applicant for contract verification, OWWA membership and OEC must secure an appointment before coming to MWO (click here to see requirements and appointment page).
